Case Management

The Case Management service links seniors to a range of community services.   Key activities include; initial comprehensive assessment of needs and personal support systems, development of an individualized service plan, coordination of the services required to implement the plan, monitoring to assess the efficacy of the plan, and regular reassessments to maintain the health and well-being of the service recipient.

Home Delivered Meals

The Home Delivered Meal program provides nutritious meals to the homebound seniors who are unable to care adequately for themselves.   The program is designed to meet the nutritional needs of those seniors who are homebound and unable able to shop for groceries or prepare meals for themselves.

Home Repair Services

The Home Repair program provides senior with access to minor home repairs and modifications to insure that they are able to continue to live safely in their own home.

In-Home Services

The In-Home Services program assists seniors to age in place by providing Homemaking, Personal Care, and Respite in the home.   The Homemaking service provides light housekeeping in the senior’s living space to maintain a clean, safe and healthy environment.   Personal Care services include bathing, grooming and other personal hygiene activities.   A respite service provides an aide to stay with a senior that needs supervision, while the caregiver conducts business outside the home.

Volunteer Services

In compliance with Title III of the Older Americans Act, this program provides for volunteer opportunities to assist in the delivery of senior services. Volunteer Services are provided through contracts with Fulton County Senior Collaborative, South Fulton Senior Services and Senior Services North Fulton.
